When filling out the form, choose the department from which the course you will apply for is opened, according to the department in which the course is offered. (Example: In the 2021 Summer School, all departments take General Physics I and General Physics II courses from the Department of Electrical and Electronics Engineering. When applying for Physics courses, you must choose the Department of Electrical and Electronics Engineering, even if your department is different.)
